    Could you use a little extra TLC for dealing with an ongoing medical issue? You’re not alone. The LiveHealthy Medical Program is here to provide coaching and support customized to meet your individual needs. It’s available to you and your family members who are at increased risk for, or who are currently living with, a chronic or serious illness such as asthma, diabetes, or hypertension.

    • Your EMC medical plan partners with you for the chronic condition management portion of the program (e.g., asthma, diabetes, etc.). Eligibility is based on medical and pharmacy claims.
    • WebMD, which administers the lifestyle management portion of the program (e.g., weight management, diet, exercise, etc.), invites participants based on individual Health Review (formally Health Risk Assessment (HRA)) results.

    A representative from your medical plan or WebMD will contact you if you’re eligible to participate in the program.

    What is the EMC LiveHealthy Medical Program?

    LiveHealthy helps you and your physician manage chronic medical conditions so you can live the healthiest, most active life possible. The program is available for all EMC employees and their eligible dependents (ages 18+) enrolled in an EMC medical plan and/or our prescription drug plan through CVS Caremark.

    If I participate, what can I expect from the program?

    Each year you’ll schedule eight or nine 10-minute phone calls with a health coach from your medical plan and/or WebMD. Get personalized recommendations about exercise, proper diet, stress management, and healthier living.

    Is this program voluntary?

    Yes. If you decide at any time not to participate, just tell the coach who calls you; you shouldn’t receive any more calls.

    How does WebMD obtain my health and contact information?

    WebMD is our partner in HealthLink and receives information based on your responses to your Health Review (formally Health Risk Assessment (HRA)). By participating in an EMC medical plan, the plan automatically shares your information with WebMD, which determines if you are eligible for the program.

    How confidential is this information? Will EMC see my personal information?

    The information is strictly confidential. HIPAA and other privacy laws require our health plans and WebMD to protect your personal health information.
